Case Management
In case management, we walk alongside our clients in their journeys, providing support and resources to help them work toward their own unique goals.
Our case managers and clients work in a one-on-one relationship to identify goals and work toward those goals. We utilize a strengths-based approach, respecting our clients as the experts in their own journeys.
A strengths-based approach fosters hope and confidence by focusing on existing abilities and talents. We promote our clients’ positive skills and lived experiences as they decrease barriers toward goal attainment. Through the process, we also examine resources that might otherwise be missed.
Case managers can support clients in securing meaningful employment, stable housing, reliable transportation, child care and more. Case managers advocate for their clients, teach life skills, help access benefits, and connect clients to additional resources.
We can also provide financial support for our clients as they work toward their goals.
